What is Rca-Root Cause Analysis?


A root cause is described as a factor that causes a nonconformity and must be permanently eliminated through process improvement. The root cause is the core issue that drives the entire cause-effect reaction that ultimately leads to the problem(s) - it's the top-level cause.

It is defined as a collective term that describes a wide variety of approaches, tools and techniques used to uncover the causes of problems. Some ROOT CAUSE ANALYSIS approaches are more geared towards identifying true root causes than others, some are more general problem-solving techniques, and others only offer support for the core activity of root cause analysis.

In summary, RCA (Root cause analysis) is a tool that helps determine what, how and why an incident occurred so that steps can be taken to prevent future events.

Additionally, RCA can be used to target system-wide improvement opportunities.

How to  perform a  Root Cause Analysis?

Basically, the following steps are followed.

1) Identify the problem. Be sure to define the problem and align it with the customer need....

2) Gather data about the problem....

3) Determine what is causing the problem....

4) Prioritize the causes. ...

5) Describe solutions to the underlying problem and implement the change. ...

6) Monitor and keep going.
